Enhancing Continuity of Care in Neuropsychological Rehabilitation and Elderly Care with Virtual Assistant
In Europe, 31% of older people face the challenges of living alone or in isolation, leading to subjective loneliness and negative impacts on their health. As the population continues to age, addressing these concerns and promoting aging-in-place with self-sufficiency becomes crucial. To improve the healthcare system and the quality of life for older adults, HosmartAI introduces the “Virtual Coach for Continuity of Care” pilot solution. This complementary system aims to support older adults in their homes or clinical centres focusing on their physical and mental health rehabilitation and prevention. Led by Fundación INTRAS, the pilot project involves collaboration from ITCL, TMA, GC, AUTH, and UM.
THE SOLUTION
The Virtual Coach for Continuity of Care serves as a screening and intervention tool, detecting and preventing cognitive deterioration while offering a tool for easy planning and monitoring. It encompasses the following components:
Functional Technologies:
- GRADIOR: Supports cognitive training, screening, stimulation, and rehabilitation. It collects data during intervention sessions and designs personalized treatments based on the level of cognitive deterioration.
- iMAT app: Enables measurement of patients’ motor capacity through smartphone camera-based assessment exercises.
- e-Pokratis smartwatch telemetry: Facilitates the measurement of vital signs, including Oxygen saturation, Temperature, Blood Pressure, and Heart Rate.
Additional Features:
- Activity Plan Editor: Allows the creation and uploading of motivational content.
- Web Integration: Provides access to data, modification of therapeutic plans, and results.
- The system also includes two horizontal features: an interaction interface with a tablet or Pepper Robot and a focus on data trustworthiness and traceability.
i-Mat Prototype: Developed by AUTH, this screenshot presents the prototype of the i-Mat app, enabling the measurement of motor capacity through pre-specified exercises using a smartphone camera.
Gradior App Screenshots: Developed by IDES (INTRAS), these screenshots demonstrate the Gradior app’s functionalities, supporting cognitive training, screening, stimulation, and rehabilitation.
PILOT STUDY GOALS AND PARTICIPANTS
The pilot study, beginning in September 2023, aims to recruit 160 older adults and involve 20 healthcare professionals and informal caregivers. By May 2024, the HosmartAI team aspires to establish a useful and well-received tool that achieves the following objectives:
Assisting Patients:
- Providing comprehensive, interactive, and cross-linked ICT-based solutions for interventions and recovery services.
- Enhancing support for independent living, promoting independence, functional capacity, health status, and overall well-being.
- Offering low-intensity follow-up for early detection of health deterioration and prompt reactions.
Assisting Healthcare Professionals:
- Enabling remote prevention and therapeutic intervention modalities for healthcare professionals such as neuropsychologists, psychologists, and therapists.
The HosmartAI team aims to arrive to the mid of 2024 with an acceptable and usable solution for healthy living, focusing not only in care, but also well-being, including healthy ageing.
Clinical Intervention Pilot Site: This pilot site serves as the location for individual and group sessions, ensuring hands-on implementation of the Virtual Coach for Continuity of Care.
